These are the Prime Evils of Hell that wielded their power as a dark, sovereign triumvirate. The Three Brothers ruled over the Lesser Four by brutal force and malicious cunning. Being the eldest and strongest of the Evils, the Three Brothers were responsible for countless victories against the armies of the Light. Although they never held sway over the High Heavens for long, the Three were justly feared by enemies and subjects alike.
In the original Diablo game, the strongest Prime Evil, Diablo, was captured by the Horadrim and imprisoned inside a soulstone. Diablo was then buried deep beneath caverns under the town of Tristram. The main story line of Diablo I follows on from there.
After his defeat, Diablo seemed totally banished, but his presence was eventually made manifest inside the mind of the hero who defeated him. Slowly, the hero who slew Diablo was corrupted and driven by Diablo to seek out his two other brothers: Mephisto and Baal. The game Diablo II is where the story picks off from Diablo I, with the player playing a pursuing adventurer, trying to stop Diablo and prevent him from reuniting with his brothers. In the proceedings of Diablo II, Mephisto is defeated and his soulstone is destroyed. Eventually, Diablo himself is defeated and his soulstone is destroyed as well. However, the storyline leaves out what happened to Baal, the third brother, and thus, Diablo II is extended by an expansion pack: Diablo II: Lord of Destruction to explain what happens to Baal.
The expansion pack, Diablo II: Lord of Destruction's storyline introduces a new mythical object into the Diablo Universe, the Worldstone, which is an ancient crystal-rock that exists to prevent the Heavens and the Hells from completely enveloping the mortal realm into their respective folds. Baal is seeking it, inside the innards of Mount Arreat. However, true to the games preceding it, Baal is defeated and the Worldstone is destroyed due to Baal's corruption of it.
As of the end of Diablo II: Lord of Destruction, all of the Prime Evils have been banished to the Black Abyss.
Diablo is the Lord of Terror, and the youngest brother. He was responsible for the ensuing of mankind with violent images of bloodlust and terror; his prime weapon being the victim's fear. He was originally defeated under the labyrinths beneath Tristram but corrupted the hero who slew him and sought out his brothers, but then was defeated in Hell, by the pursuing adventurer.
Mephisto is the eldest brother, and the Lord of Hatred. He was originally captured by the Horadrim in the deserts near the city of Lut Gholein, the same place Baal was captured as well. He was moved to Kurast and imprisoned inside a Soulstone, and was kept locked inside the Guardian Tower beneath the temple city of Travincal. His eventual corruption of the regional religious heads almost ensured his release, except he was eventually defeated by the pursuing adventurer. Being the Lord of Hatred, he is the most intensely evil brother, whose greatest weapon is his victim's irrational hatreds.

Baal is the last brother in the storyline of the Diablo games to be vanquished, and also the middle brother. He is the Lord of Destruction and was the only brother to escape the pursuing adventurer. He raised an army of demons and minions and attacked the barbarian homelands of the north, laying siege to the capital city, Sescheron, and finally besieging the last city, Harrogath. He was stopped by the pursuing adventurer and eventually defeated. Baal is the Lord of Destruction and throughout the expansion pack, Diablo II: Lord of Destruction, remains true to his title.

Lesser Evils — Duriel • Andariel • Azmodan • Belial
These are the true names of the lesser of the Great Evils. For ages uncounted each have ruled over their own domains within the Burning Hells, seeking absolute dominion over their infernal brethren. As the Lesser Four continuously vied for the control of those forces that dwelled within their realms, the Greater Three held absolute power over the whole of Hell. The Lesser Four used dark and evil measures in their quest for power, and herein begins the legend of the Dark Exile.
Lilith, Mother of Misery, Queen of the Succubi and Mother to Andariel; is not normally counted among the seven great evils, but is just as powerful.
Since the release of the 1.11 patch, only Azmodan and Belial have yet to appear in-game; Lilith appears in a quest requiring the hero to collect three keys and transmuting them in the Horadric Cube to form a portal to her realm. Lilith is also heavily featured in The Sin War Trilogy.
Duriel is one of the four Lesser Evils and the boss of the second act of Diablo II: Lord of Destruction. Not much is known about Duriel, except that he is known as the Lord of Pain.
When the adventurer arrives in Tal Rasha's chamber, Duriel yells "Looking for Baal?" Duriel is located in Tal Rasha's Chamber, which can be accessed after using the Horadric Staff on the orifice found in the real Tal Rasha's Tomb. Upon entering the Chamber, players immediately encounter Duriel.
Lore:
As the Sin War plodded along through the centuries, many in Hell began to wonder just what it was they were fighting over. The Lesser Evils of Hell were growing tired of the endless ebb and flow in the tides of mortal favor, as well as those in the elaborate schemes of the three Prime Evils - Diablo, Baal and Mephisto. The Lesser Evils longed to return to the epic days of the Great Conflict, and lay siege again to the gates of the High Heavens.
Two of the Lesser Evils, Azmodan and Belial, saw the situation as their chance to overthrow the Prime Evils and take control of Hell for themselves. The two Demon Lords made a pact with Andariel and Duriel - as well as their minor brethren - assuring them that humanity would not deter the ultimate victory of Hell. Azmodan and Belial devised a plan to end the stalemate and achieve victory in the Sin War, ultimately riding the bloody crest of the Great Conflict straight into Armageddon. Thus, a great revolution was set into motion as all of Hell went to war against the Three Brothers, seeking to send them into a Dark Exile.
Over the next two centuries, the three imprisoned brothers focused their Demonic powers upon corrupting the Soulstones that bound them to the mortal plane and began bending the will of any nearby humans. Mephisto was the first imprisoned and, consequently, was the first to break free from his bondage. His wrath against those who had put him such a compromised position was swift and merciless.
As punishment for his part in the Dark Exile that sent the Prime Evils to the mortal realm, Duriel has been left behind by Baal to guard the Tomb of Tal Rasha. Embittered by this new position of solitary servitude, the Lord of Pain has claimed the souls and blood of many bold adventurers.
Andariel is one of the four Lesser Evils, and the only female of the seven powers of hell. She is a Succubus of the underworld that was sent by Diablo to invade the Citadel of the Sisterhood of the Sightless Eye (also known as the Rogues) in order to close the passage through the mountains into the eastern lands of Aranoch, so that no one would be able to follow Diablo in his guise of the Dark Wanderer and prevent him from freeing his brother Baal from Tal Rasha's Tomb.
During the assault on the Rogue Monastery many sisters were slain and others were turned to serve the Queen of the Succubi, such as Blood Raven. The surviving Rogues fled the Monastery and became scattered throughout the wilderness, although the majority of the survivors formed the Rogue Encampment, vowing to fight Andariel and reclaim their ancestral home. She is known in Hell as the Maiden of Anguish, and is the daughter of Lilith and granddaughter of Mephisto the Lord of Hatred. Ancient Horadric texts record that Andariel and the other Lesser Evils once overthrew the three Prime Evils - Diablo, Mephisto and Baal - banishing them from Hell to our world.
With this betrayal weighing against her, Andariel's presence in Sanctuary can only mean that the forces of Hell are once again aligned behind Diablo and his Brothers. While it is clear that the Maiden of Anguish is acting on behalf of Diablo to prevent anyone from following him eastward, it is also clear that she must be destroyed if the Lord of Terror is to be stopped.
Azmodan, the Lord of Sin is a Lesser Evil, part of the Great Evils of the Burning Hells. He took part in the Dark Exile overthrowing the Prime Evils, wrongfully believing Diablo and his brothers to have hesitated in the Great Conflict. Azmodan currently rules half of Hell opposing Belial. Because of their conflicting leadership, all of Hell is currently polarised in a state of civil war.

Belial, the Lord of Lies is one of the Lesser Evils, part of the Great Evils of the Burning Hells.

Lucion, the son of Mephisto, Lord of Hatred and brother to Lilith.
History:
In ages past, several hundreds of years before the events of Diablo I when Sanctuary was still a relative young world, Lucion ruled the Triune in the guise of the Primus. As the leader of the Triune, Lucion fought a secretive and subtle war against the Cathedral of Light that would decide which of the two cults would eventually dominate the human race. Lucion's plans were eventually thwarted by Uldyssian and his Nephalem followers. Lucion stars in the first of the Sin War Trilogy by Richard A. Knaak : Birthright.
Some time after the creation of Sanctuary by the renegade angel, Inarius, the world was discovered by the Burning Hells. Realizing that a full scale invasion would inevitably draw attention of the High Heavens (which would very likely result in a heavy struggle that would devastate Sanctuary, rendering it useless to either side), the Lords of the Burning Hells opted for a more subtle approach. They devised a cult that would gradually lure its followers deeper into the darkness. Which at first seemed a harmless, peaceful cult turned out to be a dark, unholy sect devoted to the three Prime Evils. At the head of this cult stood the Primus, the pontifex maximus. However, the Primus was nothing but a façade, a disguise used by the demon Lucion.
The Triune soon grew to become one of the dominating cults of Sanctuary, being second only to the Cathedral of Light, the counter-sect led by Inarius in his guise of the Veiled Prophet. The two sects continued to vie for control over the human race but without any open warfare, as it would draw unwanted attention.Instead, Lucion and Inarius plotted against each other through subtle scheming, always anticipating their opponent's moves.
Their subtle plans were disturbed by the sudden reappearance of Lilith, a renegade demoness who had her own goals and plans. In the guise of the fugitive noblewoman Lylia, Lilith sought to awake the special powers of the humans (who were in fact the offspring of pairs of renegade angels and demons), harbouring it for her own goals. Her unwitting champion was to be Uldyssian, a simple farmer from the lands near Seram. Noticing Uldyssian due to his immense powers (after it was used to the detriment of several travelling missionaries) and quickly seeing his potential, Lucion dispatched his trusted lieutenant, the high-priest Malic to capture the farmer and his companions. However, unaware of Lilith's presence, the raiding party was overwhelmed and utterly destroyed, sending Malic back fleeing to Lucion. Realizing his sister could have returned, Lucion 'blessed' his failing lieutenant with a demon limb, allowing him to remove Lilith's guise.
After a second and much more powerful raiding party consisting of many undead morlu attacked Uldyssian and his followers near the town of Partha, but failed again (this time leaving Malic dead by the hands of Lilith), Uldyssian decided to leave the town, after seeing the onslaught. Lilith followed him, still in the guise of Lylia, but soon after their reunion Lucion's spell took effect, revealing Lilith's true form to a horrified Uldyssian. Lilith easily defeated the farmer, leaving him severely wounded. Still, Uldyssian recovered and reunited with his companions, setting out for the jungles of Toraja.
It was here that Lucion personally intervened. Disguised as a white stallion, the demon tried to lure Uldyssian with him but the latter was timely warned by his brother, Mendeln. Once again taking the form of the Primus, Lucion sought one more time to persuade the farmer to join him. After both attempts failed and Uldyssian and his gathering followers started attacking the demon, Lucion changed tactics. With relative ease the demon incinerated many of Uldyssian's followers and caused the latter's close friend Achillios to die by one of his own enchanted arrows. After seeing his comrade and followers die, Uldyssian's true powers awoke as he burst out in rage. Being unable to maintain his position, Lucion finally revealed his true form: a horrid, reptilian demon with hooved legs and three tails, covered in thick scales. But even as a mighty demon, Lucion turned out to be overpowered by the sheer force of Uldyssian's abilities. As a last desperate act the demon even offered his obedience and subservience to the farmer, but all for nought. Instead of killing the demon, Uldyssian simply imagined the demon to be nothing, and so the great Lucion faded away and disappeared.
It is unknown what exactly became of Lucion. Though the first book of The sin war is clear on the fact that he was not 'killed' but simply 'was no more', it is never explained what that meant. It could very well be that Lucion was simply banished back into the Burning Hells where he would have to answer to his father Mephisto...or maybe Uldyssian's awesome display of force even banished Lucion to a plane beyond the reach of both Heaven and Hell. But in the second book of The sin war is clearly explained that Uldyssian assumed that when a demon dies it not longer exists, as he said that had been the fate of Lucion, implying that the demon may indeed have died.

Kabraxis is a demon mentioned in The Black Road. He is known as the Thief of Hope. Many of his worshipers would commit the most heinous sins for three days, and their sins would then be washed away in the symbolic blood of Kabraxis. He was the demon that ended up possessing Darrick Lang on the Black Road, after Darrick decapitated him. The whereabouts of this crafty demon are unknown at this juncture. Another point about Kabraxis is that he was an enemy of Diablo and his brothers. He was also known as Ice-claw the Merciless to the barbarian hordes of the frozen North.
The Ancient Evil:
Kabraxis has been upon Sanctuary just as long as the Prime Evils. It was not until the runic portal of his black road was unearthed that he returned from an unknown time of banishment. He has been known by many names and often times hides behind an order seeking to heal the sick or ease the troubles of humanity. Yet behind those benevolent groups lurked his scheming plots of corruption. However since his release and supposed defeat the whereabouts of Kabraxis have been left unknown.
Xazax is a mantis-like demon featured in the novel Legacy of Blood who is apparently blood bound to the sorceress Galeona.
He attempts a coup with General Augustus Malevolyn, who is in possession of Bartuc's helmet, in order to gain Malevolyn as a puppet in Bartuc's armor. Galeona is later killed by Norrec Vizharan, who is under the possession of Bartuc's armor, but Xazax remains on the mortal plain for reasons unknown. Xazax is also responsible for turning Malevolyn's mercenary army into demons in men's skin, which are later banished by Norrec. Later during the fight between Norrec and Malevolyn, Kara Nightshadow stabs Xazax in the back with her ivory dagger leaving a wound that would not heal, and also cast a spell upon Xazax the produces illusions of angels and demons he fears, forcing him to run in fear.
As he runs through the Desert trying to escape the illusions, he realizes one voice he hears is real, turning around he manages to utter "Diab-" before screams of terror are heard. This presumably is the lord of terror, Diablo that found him and is left off that he had killed the creature. This end event sets the book just before the heroes in Diablo II arrive in Lut Gholein.
Astrogha was a favoured servant of Diablo, summoned into Sanctuary by Lucion to assist with casting a spell on Lilith. Being far more intelligent than his destructive colleague Gulag, Atrogha was a cunning and plotting demon, always scheming and planning in the dark recesses of the Temple of the Triune. He is featured in both The Sin War Trilogy and the Moon of the Spider by Richard A. Knaak.
History:
The Sin War
Astrogha appears as a giant, humanoid spider. His lower body is that of a spider, but his torso and head resemble something vaguely human. On his head he has several hand-like limbs, used for pushing prey into his mouth. He also has many smaller spider-spawns made of his essence, called 'Children', who act as spies. Astrogha also has the ability to create thick webbing which he uses to constrict prey in. He feeds on human bodies, sucking out their fluids.
Astrogha is a somewhat reluctant ally of Lucion, though he always remains true to his lord, Diablo. Astrogha once plotted to overthrow Lucion, but realizing his master would not back him against Mephisto, Lucion's father, he backed off. After Lucion's strange disappearance at the end of the Birthright novel, Astrogha once more planned to take his place as the Primus, but the strange reappearance of the Primus (this time Lilith) made him abandon his plans. After Astrogha finally realized Lilith was now using the guise of the Primus, Astrogha started scheming against her, even taking the form of the Primus himself for a while. After a short showdown with Lilith, which was only a way to gauge her powers, Astrogha fled the Temple, once again opting for scheming in the shadows.
Moon of the Spider
Appearing once again as a main character of the book, Astrogha remains in a orb called the Moon of the Spider. A prison bound to an unknown dimension, Astrogha was sent there by Vizjerei after his own cult fell. He was then released by a Noble of Westmarch by the name of Aldric Jitan,which Astrogha then tries to lay waste to the capital through his 'Children'. With was only though the efforts of the Necromancer Zayl and the Noble Salene Nesardo, who is revealed to be a descendant of Astrogha himself, that Astrogha was banished back into the Moon.
Assur was a baron of The Burning Hells. The favored of the Lord of Terror, Diablo. He had bright and glowing red eyes, and horns protruded from his shoulders, elbows and knees. He wore a primitive loincloth, a belt and a giant sword. On his chest was an emblazoned glyph, a blessing from Diablo that can only be cast once a millennium that made Assur invulnerable to the hand of any living thing.
Assur was slain by Siggard at the city of Brennor after Assur's army had initially undertaken a successful siege. Demonsbane puts the year of this battle as 302, and it is considered one of the first great battles of The Sin War.
Gulag was a dimwitted but powerful minion of Baal. He was summoned by Lucion to assist him in casting a spell upon Lilith. Gulag appears in the first two novels of the Sin War series by Richard A. Knaak.
Gulag doesn't seem to have a more or less standard appearance like Astrogha. Instead, Gulag appears as a liquid mass of stone with only a huge gaping mouth on top. The stone mass can take the shape of a human, with only the mouth serving as a head. Like his master, Gulag revels in mindless destruction. During a showdown with Uldyssian, he was eventually overpowered by the farmer, who literally tore him to shreds. In the end, all that was left of Gulag was a stinking puddle.
Na-Krul was a demon that was imprisoned by Diablo himself. He is the boss of the expansion of Diablo, Hellfire, made by Sierra. This creature was one who served as Diablo's chief minion and strongest ally, but soon Na-Krul and his cohorts were banished to the Nether Realms when he incited rebellion among Diablo's minions. Freed by an evil sorcerer, Na-Krul then set up command in the ancient Demon Crypts and once again marshaled his plans to destroy Diablo and rule the Burning Hells for himself. Gathering together a small army of never-before-seen monsters, Na-Krul possessed a threat nearly as deadly as the mighty Diablo.
The player then travelled deep into the Demon Crypts and released him from his prison within his ritual chamber. Battle ensued, in which the player was able to defeat the demon.
